It seems the ASSOCIATION - what media player kicks in to play which files may be goofed up. This is possible because many players can play several types of files - often one's from competitors. (i.e. RealPlayer can play back Windows AVI or QuickTime MOV formats - and during setup can ask if it can 'hijack' these formats and become the player to handle them.)

You might try not only reinstalling the latest version of Flash Player, then downloading the latest QuickTime installer - also Flip4Mac, RealPlayer, and the older Windows Media Player to have every tool you need.

If that doesn't help, then locating and trashing every LIBRARY > PREFERENCES file related to all these media players _may_ reset them to defaults.

Worst case you might have to to do a Clean Archive Reinstall of your system.
