POP UP asking for Facebook password

i keep getting a pop up asking for my Facebook password. I have tried everything I can think of to get rid of it but it keeps coming back every few minutes. HELP

-Is this happening in Safari or another browser?
-Can you login to Facebook on another computer?
-Do you have issues with any other passwords?
