Add Different Speech Commands

I want my mac to be able to recognize and obviously do these commands:

"Next track" - In iTunes
"Previous track" - In iTunes
"Volume up/down" - In iTunes and Safari
"Refresh page" - In Safari

by speech though, is there any way I can add commands ?
Any and all help is appreciated
